Care Manager, CMA or CNA
Summary
As a Care Manager, you will coordinate care for Medicare patients with chronic conditions through monthly telephone calls. Your role involves ensuring effective communication and high-quality care by helping patients navigate their healthcare needs. Care Manager's are part of a care team, providing VIP support to improve the lives of patients and improve health outcomes.
Key Responsibilities:
- Conduct monthly patient outreach via phone to manage chronic conditions, provide education on preventive care, and assist in navigating their healthcare system.
- Act as a liaison between patients, healthcare providers, care team, and support services
- Maintain accurate and up-to-date patient records
- Assist in developing personalized care plans
Qualifications:
- Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA)
- Or Certified Nursing Assistant
- Strong organizational and multitasking skills
- Excellent communication and interpersonal abilities
- Proficient in using electronic health records (EHR) systems
- Experience in care coordination or a related field is preferred
