Carson Medical Group now offers Chronic Care Management (CCM), a tool available to Medicare patients living with more than one chronic condition. This convenient and streamlined program provides additional support from our dedicated team between your regular office visits.
When you enroll in the CCM program, you will be appointed an official Care Manager who will work hand-in-hand with your provider to develop a personalized care plan tailored to your specific needs and health goals. Your Care Manager will assist with managing your day-to-day health needs and coordinate various aspects of your care, such as specialist visits, medications, and symptom management. Think of your Care Manager as your trusted advocate, taking the guesswork out of your healthcare journey!
The CCM program is available to Carson Medical Group patients receiving Medicare benefits who are living with two or more chronic conditions. These conditions can include high blood pressure, high cholesterol, arthritis, diabetes, depression, Alzheimer’s, cancer, asthma, and more.
Managing multiple chronic conditions can be overwhelming, but our qualified and empathetic team is dedicated to providing effective and coordinated care, working closely with all parties involved in your treatment.
Medicare covers most of the costs for this service—and if you have a Medicare supplement plan, it might even be covered at 100%!
